STAR Center Full Time Employees - Pre-Return To Work Questionnaire
All STAR Center full time employees who have traveled outside the tri-county area or visited a medical facility as detailed in the AMO Plans HR policy MUST complete and submit the following questionnaire prior to their return to work. Authorization MUST be received from either the Director of Training / Director, Operations or HR before access to the campus is allowed.
